MEMBERSHIP CARDS
| As required by Supreme
Law VI, Section 21, the chapters shall require all
persons elected to membership to sign uniform membership
record cards to be supplied by the National Headquarters.
These cards are kept on file at the national headquarters forever. The white copy of the cards must be mailed to the Headquarters with fees within three (3) days of the ceremony. Gold cards are to be kept on file at the local chapter. New Membership Cards are available at the Pi Kappa Phi National Headquarters. |

Fill out BOTH the white and gold copies of the 3.5" x 5.5" pre-initiation cards.
MAIL the white copy to the Headquarters with $65 for each pre-initiation card and keep the gold copy for your chapter records. |

Fill out BOTH the white and gold copies of the 5"x8" initiation cards.
MAIL the white copy to the Headquarters with $185 for each initiation card and keep the gold copy for your chapter records. |
